Posted below are links to several forms which are required for various events scheduled throughout the year. Please check this page for updated forms for upcoming events.
The forms below must be completed and returned in order to be considered for the mission trip to Mexico this summer (July 15-22). Please make sure to read all of the Information Packet before filling out your application. Applicants must have a passport to apply. Forms may be submitted starting in February. Please return application in the silver box outside the Student Minstry office. If you have any questions please contact Kerry White, 470-9800.
